Steps to author Doctor Dicussion Guide
Step 1: Navigate to the page where you want to add the component. Click on "Drag components here" and click on + to add the container on page.

Step 2: Search and select "Survey Container" from dropdown list and then drag and drop it onto the desired location on the page.
Step 3: Once the container is added, you can configure it by selecting configure icon as showcased in below image.
Step 4: In the component configuration dialog, you can author required properties.

Check this to get Normal Email - On check of this checkbox below, 4 fields (will show) are added; if unchecked, the checkbox 4 fields will get hidden.

Step 5: Navigate to the Survey container, Click on "Drag components here" and click on + to add the component inside the container.
Step 6: Search and select Survey Questions Component from dropdown list and then drag and drop it onto the desired location on the page.
Step 7: Once the component is added, you can configure it by selecting configure icon as showcased in below image.
Note : Each survey questions component act as a step in form.
Step 8: In the component configuration dialog, you can author required properties.

Step 9 : After authoring Survey container and Survey questions component, Create a page using the "Build Easy Pdf Page Template".
Step 10 : In the page, copy the Survey container which is already authored in the previous page. After adding, enable the 'Check to use this component for answers' checkbox in Survey container and enable 'Step answers component' checkbox in Survey questions component.
Step 11 : Submit the form, By selecting the answers to the questions as shown below.

Step 12 : Aftersubmitting the form, We have 2 options.
1) We can download the form in the pdf format by clicking on the Download now button.
2) We can send email, by clicking on the Send Email button.
